• Write a check payable to San Francisco Study Center (CAMK2 Therapeutics Network’s fiscal sponsor). The Study Center is a 501(c)(3), tax-exempt nonprofit organization, Tax ID: 94-2168838
• IMPORTANT: On the memo line of the check, indicate that the donation is for CAMK2 Therapeutics Network
• Include a note with your name, mailing address, telephone and email address and any other instructions or information.
